Referrals
If you are a healthcare or wellness provider with a patient/client who is interested in nutrition counseling, we are happy to assist with the referral process.
In-person appointments are available at our office located in downtown Vancouver, Washington for your clients in the Portland/Vancouver area.
Telehealth appointments are available for both residents of Washington and Oregon, as well as Arizona, California, Colorado, Kansas, Michigan, North Carolina, Vermont, and Virginia. Note, for residents of California, a referral from a prescribing physician must be received for medical nutrition therapy per state law.
You can directly contact us to coordinate care and we will send you a referral form if needed. Depending on any insurance billing requirements, we may need a formal written referral.

You can refer adults, 18 years or older. Please refer children to a pediatric dietitian. For patients with a suspected or an active eating disorder, please refer to dietitian who is a specialist in eating disorders. We can see patients needing nutrition counseling who are in remission from an eating disorder.
We can see patients at our office in Vancouver, Washington.

Common nutrition-related medical diagnosis including prediabetes, diabetes, cardiovascular diseases and risk factors (hypertriglyceridemia, hypercholesterolemia, hypertension), malnutrition (underweight, protein deficiency or other nutrient deficiency), gastrointestinal disorders, celiac disease, chronic kidney disease, and overweight/obesity.
For patients with dysphagia, please refer first to a speech language pathologist to determine food texture and liquid viscosity needs. Once that is established, diet instruction can then be given for their dysphagia and other dietary needs.

We are in-network with Medicare, which only covers medical nutrition therapy for diabetes, chronic kidney disease and post-kidney transplant with some parameters.
We are also in-network with Aetna, Cigna, Curative, First Health, Lifewise of WA, Premera Blue Shield and Regence Blue Cross Blue Shield, Medicare, and Molina. Covered services varies by specific plan.
As we continue to grow our insurance network, we will update our list of insurances. For any patients whose insurance we are not in-network with opting for self-pay, we will provide a super bill which they can chose to submit to their insurance for potential reimbursement.

Yes, for self-pay clients we offer a sliding scale application for a reduced rate. Eligibility is dependent on proof of income.

Our HIPAA compliant referral form is also on this webpage to get the process started. Depending on insurance requirements we may need to send you a prior authorization request or request medical records for proof of medical history.
